COVID-19 is an extremely contagious virus that spreads easily from person-to-person contact. The COVID-19 pandemic has changed the way in which workplaces must operate. Gilmour Photography has implemented preventative measures to reduce the spread of COVID-19.
Gilmour Photography has created a COVID-19 Health and Safety Workplace Policy in line with current Alberta public health guidelines. This policy is designed to reduce and prevent the spread of COVID-19. However, we cannot guarantee that you will not become infected by COVID-19 on set. Furthermore, participating in this shoot could increase your risk of exposure to COVID-19.
By signing this agreement and participating in the shoot, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that participating in this shoot may expose me to COVID-19, a virus that could cause illness, serious health complications, and even death.
I voluntarily agree to assume all risks associated with participating in this shoot with respect to COVID-19. I hereby release, discharge, and hold harmless Gilmour Photography from all claims relating to my participation in this shoot.
I further agree to immediately alert Gilmour Photography if I display any known symptoms of COVID-19 or other unusual and inexplicable health-related symptoms within 14 days of the shoot.
